Notice Regarding Form 1095-C Availability (2025 Tax Year)

As of 2025, employers are no longer required to automatically send employees a Form 1095-C unless the employee requests a copy of the applicable form. Centurion Surgery Centers will continue to generate and file all required Forms 1095-C with the Internal Revenue Service (IRS) in a timely manner, as required by law.

How to Request Your Form

If you would like to receive a copy of your 2025 Form 1095-C, you may request it by contacting:

Posting Period
